What is semaglutide ?

What are the medications? 

These medications contain Semaglutide and are marketed under different brand names for the treatment of type 2 diabetes and weight loss.

How does it work (mechanism of action) for weight loss?

Semaglutide’s mechanism of action for weight loss is thought to be multifactorial but primarily involves its effects on the appetite control centers in the brain.

  1. Semaglutide is a GLP-1 receptor agonist, which means that it activates GLP-1 receptors in the brain. These receptors are located in areas of the brain that regulate appetite and food intake, including the hypothalamus and brainstem.
  2. Activation of GLP-1 receptors in these areas leads to a decrease in appetite, an increase in feelings of fullness, and a reduction in cravings for high-calorie foods. This can lead to a reduction in food intake and ultimately, weight loss.
  3. In addition to its effects on appetite and food intake, semaglutide may also increase energy expenditure and improve insulin sensitivity, which can contribute to weight loss.
  4. Activation of the GLP-1 receptor by semaglutide leads to increased insulin secretion by pancreatic beta cells in a glucose-dependent manner, meaning that insulin is only released when blood glucose levels are elevated. This helps to reduce blood glucose levels and improve glycemic control in patients with type 2 diabetes.

Semaglutide for weight loss is administered as a subcutaneous injection once a week, and is currently approved for use in adults with a body mass index (BMI) of 30 or greater, or a BMI of 27 or greater with at least one weight-related condition such as high blood pressure, type 2 diabetes, or high cholesterol.

What is the evidence behind Semaglutide use and weight loss?

The New England Journal of Medicine published the results of the Semaglutide Treatment Effect in People with Obesity (STEP) clinical trials, which investigated the efficacy and safety of semaglutide for weight loss in adults with obesity.

In the STEP 1 trial, which involved over 1,900 adults with obesity or overweight and at least one weight-related condition, participants who received semaglutide once weekly had significantly greater weight loss compared to those who received placebo. At week 68, participants in the semaglutide group had an average weight loss of 14.9% compared to 2.4% in the placebo group.

Similarly, in the STEP 2 trial, which involved over 1,200 adults with type 2 diabetes and overweight or obesity, participants who received semaglutide had significantly greater weight loss and better glycemic control compared to those who received placebo. At week 68, participants in the semaglutide group had an average weight loss of 9.6% compared to 3.4% in the placebo group.

The results of the STEP trials demonstrate that semaglutide can be an effective treatment option for weight loss in adults with obesity or overweight and at least one weight-related condition, as well as in adults with type 2 diabetes and overweight or obesity.
